Date, place and, as closely as you know it, time of birth, with the thing you actually want to ask about.
He prepares
The chart, the divisional charts and the daśā periods are studied in advance, against the question you have raised.
You speak, he reads
You describe what is happening in your own words. He reads the chart against it and shows you the reasoning — which placements, which periods, and why.
You receive it in writing
A written note within 3 days: what was discussed, what your chart indicates, and what, if anything, it calls for.
